Sole Proprietor and/or Partnership and/or Limited Liability Partnership Limited Company and/or Plc Companies House documentation confirming creation of a limited company or Plc, the names of the directors and shareholders. If there are shareholders and/or key people, in addition to the previous sole proprietor, the unknown individuals will need to complete Annex A and/or PML applications and provide all supporting documentation. If the limited company and/or plc has 4 or more people in positions of management responsibility all of them will require PMLs, including the original sole proprietor.
Sole Proprietor Partnership and/or Limited Liability Partnership A copy of the partnership agreement signed by all parties. If any of the partners are unknown to the Gambling Commission, they will need to complete Annex A and/or PML applications and provide all supporting documentation. If the partnership has 4 or more people in positions of management responsibility all of them will require PMLs, including the original sole proprietor.
Partnership and/or Limited Liability Partnership of 2 People Sole Proprietor Written confirmation from both partners that the partnership has been dissolved for business reasons. If confirmation cannot be provided by the departing partner a statement from a solicitor or accountant will be acceptable.
Partnership Limited Liability Partnership A copy of the new partnership agreement signed by all parties. If a new partner has been introduced, they will need to complete Annex A and/or PML applications and provide all supporting documentation. If the new partnership has 4 or more people in positions of management responsibility all of them will require PMLs.
Limited Company and/or Plc Sole Proprietor Companies House documentation confirming that the company has been dissolved. N/A
Limited Company and/or Plc Limited Liability Partnership and/or Partnership Companies House documentation confirming that the company has been dissolved.
