Licensing authority forms

It is a statutory requirement that applicants use the correct forms to give proper notice of applications, variations, and so on to all responsible authorities, including the Gambling Commission.

Licensing authorities must also use the correct statutory forms for the grant and/or rejection of applications (new, variations, transfers, and so on).

You also have statutory duties to notify the Commission as well as the applicant and other responsible authorities of the grant and/or rejection of applications (new, variations, transfers, and so on) as well as the revocation, surrender or lapse of a premises licence using the correct statutory forms and notices.

Part 3, s12 and 13 of the Gambling Act 2005 (Premises Licences and Provisional Statements) Regulations 2007 (opens in new tab)

We have brought all the statutory forms (both in English and in Welsh) into one place to help you to comply with those statutory processes.

The statutory forms and notices which apply in Scotland are subject to different Regulations and these can be found on the separate page for Scotland.
