Mae’n rhaid i awdurdodau trwyddedu hefyd ddefnyddio’r ffurflenni statudol cywir ar gyfer derbyn a gwrthod ceisiadau, yn cynnwys ceisiadau newydd, amrywiadau a throsglwyddiadau.

Mae gennych ddyletswyddau statudol hefyd i hysbysu’r Comisiwn Hapchwarae yn ogystal â’r ymgeisydd ac awdurdodau cyfrifol eraill ynghylch derbyn a gwrthod ceisiadau yn ogystal â diddymu, ildio neu ddirymu trwydded eiddo gan ddefnyddio’r ffurflenni statudol cywir.
