Key events that must be reported
Details of the key events that must be reported to the Gambling
Commission, by personal licence holders, are set out in a licence
condition and can be found on page 5 of the document
Conditions and Codes of Practice applicable to Personal Management
Licences/Personal Functional Licences enclosed with your
licence. Key events for personal licence holders include:
- the imposition of a disciplinary sanction against you,
including dismissal, for gross misconduct
- your resignation from a position for which a personal licence
is required following commencement of disciplinary proceedings in
respect of gross misconduct
- your disqualification from acting as a company director
- the entry of any court judgment against you
- the presentation of a petition for your bankruptcy or
sequestration or entering into an individual voluntary
agreement
- your conviction for any criminal offence or receipt of a formal
police caution or any other out-of-court disposal
- the imposition of any sanction or penalty against you following
an investigation by any professional, statutory, regulatory or
government body
- a change in your name or address.
When reporting key events you must provide the following
information:
- the date the key event took place
- a description of the key event
- the outcome of the key event.
In addition, for key events involving a change of name or
address we will require the following:
- proof of you new name, for example, a marriage certificate
or deed poll
- proof of your new address, for example, a bank statement
or utility bill.
If you are unable to provide proof of the change of name or
address immediately you should still report the key event to us and
confirm when you expect to be able to forward the relevant
documentation.
When should key events be reported?
Licence holders must notify us of any key event as soon as
reasonably practicable and, in any event, within five working days
of becoming aware of it.
