What are the total licensing costs for gaming machines (fruit machines, slot machines)?

Operator licence application fee (payable at the
time of application)
Operating licence annual fee (your first
annual fee is payable 30 days after your operating licence is
issued)
Personal management licence fees (you must pay a
fee for each application you make)
