Licensing authority forms

It is a statutory requirement that applicants use the correct forms to give proper notice of applications, variations etc to all responsible authorities, including the Gambling Commission.

Licensing authorities must also use the correct statutory forms for the grant/rejection of applications (new, variations, transfers etc).

You also have statutory duties to notify the Gambling Commission as well as the applicant and other responsible authorities of the grant/rejection of applications (new, variations, transfers etc) as well as the revocation, surrender or lapse of a premises licence using the correct statutory forms.

Part 3, s12 and 13 of the Gambling Act 2005 (Premises Licences and Provisional Statements) Regulations 2007

We have brought all the statutory forms (both in English and in Welsh) into one place to help you to comply with those statutory processes.
